Scheduling Appointments:

For Established Patients:
If you are an established patient, please call GWCIM at 202-833-5055 to schedule follow-up appointments.

For New Patients:
Welcome! Here’s a step-by-step guide to help you schedule your first appointment:

• New Patient Inquiry:
  Please start by completing the quick New Patient Inquiry form at the bottom of this page.
• ChARM Patient Portal Invitation:
  After submitting the inquiry, we will email you an INVITATION to set up your account in the ChARM Patient Portal. All forms and further communications will be handled securely through the ChARM EMR (Electronic Medical Records) system.
• Hello Message to Managers:
  Once your ChARM Patient Portal is set up, send a HELLO message to our managers, Monica Arias and Cherrie Postigo, under the messages section in your ChARM Portal.  You can login into ChARM Portal at any time HERE. For immediate assistance, you can also call 202-833-5055 or email info@gwcim.com.
• Forms Submission:
  In the ChARM Portal, Monica and Cherrie will send you two forms: the Patient Health Inventory (PHI) Form and the Informed Consent Form package. You can also upload any relevant recent medical documents.
• Review by Medical Director:
  Your forms will be reviewed by our Medical Director, Dr. Mikhail Kogan, to ensure the best match with an intake specialist or a specialist of your choice. This review will be done before your first appointment, optimizing your time with the provider.
• Appointment Scheduling:
  We will call you to set up your INITIAL appointment.
• Initial Appointment:
  During your initial appointment, you will:
  • Discuss in-depth your concerns with an intake provider.
  • Undergo a medical exam if needed.
  • Develop a treatment plan.
  • Receive recommendations and referrals to other specialists if needed.
  • Have laboratory tests ordered if needed.
• Follow-Up Appointments:
  Follow-up appointments will be scheduled based on individual treatment plans. Your medical care will always be coordinated by your intake provider, even if you were referred to see other practitioners.

Cancellation Policy:
When you need to cancel an appointment, we appreciate your consideration for our providers’ time. No charge for cancellations made more than two business days prior to the appointment. Late cancellation (less than TWO business days) or no-show fee is 100% of the provider’s fee.
