Emergency Care Notice:
GWCIM does not provide Emergency Care. In case of any emergency, please call 911.

Fees and Policies:
Before proceeding, please review the information on our Fees and Policies. GWCIM does not participate in any insurance plans.

Important!:  After registering for our ChARM Patient Portal, all communications  MUST be done through the Patient Portal.  It is important not to miss any messages in the Portal.

Scheduling Appointments:

Established Patients:  please call 202-833-5055 to schedule follow-up appointments.

New Patients: Here’s a step-by-step guide to help you schedule your first appointment:

  • Fill out New Patient Inquiry (NPI):
    Please start by completing the quick New Patient Inquiry at the bottom of this page.
  • Receive ChARM Electronic Health Records Patient Portal Invitation with E-Forms (Informed Consent and PHI: Patient Health Inventory):
    In 2-3 days, you will receive an INVITATION email to set up your account in the ChARM Patient Portal and fill out required forms.
  • Submit them as soon as possible
  • Portal Access: You can log into the ChARM Patient Portal any time HERE.  All further communications will be done ONLY through the Patient Portal.
  • For immediate assistance, call 202-833-5055.
  • Additional Medical Records Submission (in Portal):
    You can upload any relevant recent medical documents, records, labs, referrals, and images.
  • Reviewed by Medical Director:
    Our Medical Director will review your forms and match you with an intake specialist or a specialist of your choice.
  • Appointment Scheduling:
    We will contact you to set up your FIRST appointment.
  • During your first appointment, you will:
    • Discuss all your concerns with an intake provider.
    • Review some of your previous medical history and records.
    • Undergo a medical exam when needed.
    • Discuss possible referrals to other specialists to create a medical team.
    • Discuss recommendations and have the Lab tests ordered.
    • Develop an individualized treatment plan together with your intake provider.

Cancellation Policy:

There is no charge for cancellations made more than two business days before the appointment. Late cancellation (less than TWO business days) or no-show fees are 100% of the provider’s fee.
